Payroll Deducation and Leaves of Absence Q&A

What about Payroll deduction for Flight Attendants with Aflac policies that signed up prior to 2011?
This Payroll deduction project only affects the Aflac/CAIC Group policies including Short Term Disability, Critical Illness, Accident, and Hospital Indemnity, signed up for in 2011 onward. All other prior United AFA Aflac polices will stay on a direct pay basis.

What happened to the administration fee of $1.52 per policy?
All participating members will see reduced monthly premiums, as the third party administrator BRMS, will no longer be collecting premiums.  This is the advantage of Payroll deduction. 

I am retired, or planning on retiring or leaving United. I would like to continue my policies with the portability provisions.  What do I need to do?
Contact Aflac Customer Service at 1-800-433-0336 to set up direct payments with Aflac. 

I am on a Medical Leave of Absence and no longer have a paycheck coming in from United.  How long do I have to contact Aflac to set up direct payments before my policies are cancelled?
You should make arrangements with our broker Tina Jeffrey within 30 days. The following instructions are for making check payments for missed premiums, payments cannot be made over the phone:

Please make a check out to “Aflac”

  • In the memo line include your 6 digit file number
  • If more than one premium payment is owed, monthly payments in equal amounts 
  • Send the check payment to the Design Benefits Inc. Agency listed below:

Can I suspend my policies while I am on Leave of Absence, not make payments, and then restart them once I return to work?
No.  You should make arrangements to pay premiums with our broker Tina Jeffrey as soon as possible:

(888) 772-1144 ext 105 or tina@designbenefitsonline.com.  Some policies, such as Short Term Disability (after 60 days) have waiver of premium while you are taking a benefit.

Where can I go to get more information regarding Aflac/CAIC policies and claim forms?
An AFA-Aflac supplemental insurance website is available from www.unitedafa.org.  Go to “Pay and Benefits” tab, and then Aflac.
