The Process to File an Allstate Benefits Claim

Flight Attendants who signed up for Allstate Voluntary Supplemental Insurance Benefits during the open enrollment window are reminded that Design Benefits offers a wide array of resources via a portal located on unitedafa.org. Flight Attendants may review their policies, certificate of coverage or file an Allstate claim by following the step-by-step prompts listed below: 

1. To submit a Claim or view your Certificate of Coverage, please refer to www.unitedafa.org Union website. Next, you will hover over the “Pay and Benefits” tab located at the top of this page, select “Voluntary Supplemental Insurance Benefits” and then choose the Design Benefits “visit website” blue and white button.

2. Next, select the “Allstate Benefits Claim Site” Button. 

3. The first time you are on the website, you must register for claims and policy access. When registering, use the last four digits of your unique employee ID numbernot the last four of your SSN.  

Additionally, Design Benefits offers a “How to use the Portal “ video via Unitedafa.org.Flight Attendants needing additional information are encouraged to review this resource.
