Initiating AFA Supplemental Benefits Claims

DEC 13 - As a reminder, United management is not involved in completing the “Employer Section” of the claim form.  We’ve devised a process whereby the Union provides information needed to process your claim utilizing the staff at the MEC Office in Chicago. 

 

Our MEC Benefits Committee reminds all of our Members that when filing a claim for any of our AFA Supplemental Benefit products with CAIC (administered by NGP) or Allstate (administered by Design Benefits), your best reference source is our MEC website where you can easily locate directions on how and with which company to file a claim.

To obtain information to file a claim with Design Benefits or NGP, from the home page of the MEC website select Committees >> Benefits >> Insurance >> Insurance (Supplemental).
