As part of our Contractual Attendance and Performance management provisions of our Contract, we negotiated an opportunity for Flight Attendants to receive a ½ point reduction to the Attendance points assessed for absences due to illness of injury that are six (6) calendar days or less.

As part of our Contractual Attendance and Performance management provisions of our Contract, we negotiated an opportunity for Flight Attendants to receive a ½ point reduction to the Attendance points assessed for absences due to illness of injury that are six (6) calendar days or less. In order to qualify for this ½ point reduction, Flight Attendants are required to obtain from her/his treating physician (or a physician affiliated with United’s medical department) a written note that includes the following required information:
- Date of illness
- Date of examination
- Date of return to work
- Signature of the physician or the physician’s designee on a document that contains, at a minimum, the physician’s printed name, work address, and work telephone number.
Because Flight Attendants are not required to disclose the nature of a personal illness or injury to anyone other than United Medical, only the information above is required on the physician’s note. If, however, you obtain a document that contains any confidential medical information (diagnosis, prognosis, condition, after treatment care, etc.) be sure to black out that information before presenting the physician’s note to your supervisor or designated personnel in the base.
Physician’s notes are to be provided on or before your next scheduled pairing or Reserve assignment and may be presented either in person of by e-mailing a scanned copy or photo of the physician’s note
