Occupational Injury Resources
General Documents
Know Your Benefits will summarize and compile the Contractual benefits from the current JCBA for you to review.
Immediate Actions is a chronological step by step for you to review post-injury.
Initial Visit
On your first visit to your Medical Professional, take the following documents. These are also located on the United Flying Together site under Workers Compensation Guide.
Release Forms
United AFA-CWA MEC recommends using the following Medical Records Release Form if asked by Sedgwick to provide one for additional medical documentation.
United Flight Attendants may request United AFA-CWA to provide a list of vetted Worker's Compensation Attorneys if necessary.
Loss of Hours due to Occupational Injury/Illness
If you lose time for your injury/illness, and call in sick, we suggest that you complete the Pay Options Acknowledgement letter via Flying Together -> Help Hub -> Employee Services -> Pay -> Submit a Pay Option Acknowledgement Letter (Occupational).
Individual financial circumstances may vary, so we recommend that you consider choosing one of the pay options that allow you to use your sick leave bank to supplement your income while
your claim is being investigated.
